Intuitive Surgical: Barclays Sees a 2027 Turnaround While Cramer Balks at the Multiple

Barclays has kicked off coverage of Intuitive Surgical with an Overweight rating, and the timing is telling. The British investment bank expects the broader medtech sector to find its footing from 2027 onward, with fundamental conditions steadying and investor sentiment brightening alongside them. That vote of confidence lands as the robotic-surgery pioneer trades well below its former highs, leaving shareholders to weigh a sturdy operating story against a valuation that has long invited second-guessing.

The stock’s recent path has been uneven. On Tuesday it closed 0.8% lower at EUR 359.45 on European venues, yet Wednesday brought a modest rebound of 0.9% to EUR 362.55. Zoom out and the picture darkens: the shares have shed 27% since the start of the year. Even so, the past 30 days have delivered a 14% countermove, a recovery that has taken some of the sting out of this year’s decline.

Procedure Growth Anchors the Investment Case

What keeps the bulls engaged is the steady climb in clinical volumes. During the second quarter of fiscal 2026, procedures performed with da Vinci systems rose 16% year over year worldwide, and adjusted earnings per share for the same period came in at USD 2.80. Management has guided toward roughly 14.5% procedure growth for the full year 2026 — a figure that tracks the number of surgeries performed and should not be confused with overall revenue growth.

The installed base is shifting generations, which matters for future volumes. Of the 468 systems placed in the second quarter of fiscal 2026, 246 were the newer da Vinci 5 generation. That replacement cycle underpins the procedure forecasts the company is banking on.

Recurring revenue does the heavy lifting financially. Instruments and accessories accounted for USD 6.0 billion of the USD 10.1 billion in total revenue booked in fiscal 2025, since every robot sold to a hospital generates a stream of follow-on orders for disposable and reusable components across its working life. Clinicians should note, however, that management plans to cut usage fees for selected instrument groups starting in the first half of 2027. The company has not yet quantified what that pricing change will mean for margins.

A Rich Multiple Draws Fire

Second-quarter results showed the business in fine fettle. Revenue climbed 18.4% year over year to USD 2.89 billion, and earnings per share beat the analyst consensus. Gross margin reached 66.68%, the operating margin stood at 33.60%, and the company holds USD 5.22 billion in cash with no debt. Demand for minimally invasive systems continues unabated across clinics worldwide.

Even so, skeptics argue the valuation has run its course. A historically elevated price-to-earnings ratio means the market has already priced in plenty of future success, raising the stakes for any stumble. Jim Cramer, host of CNBC’s Mad Money, advised against opening a new position on Tuesday’s broadcast. He acknowledged the company’s operational strength but called the stock too expensive at current levels, singling out mounting competition from medtech heavyweights such as Johnson & Johnson as the key risk.

Management has been trimming exposure as well. EVP Gary Loeb sold 400 shares on August 10, and VP Fredrik Widman disposed of 1,089 shares on September 15, bringing total insider sales for the quarter to 12,370 shares. Institutional flows have shifted too: Eastern Bank cut its stake substantially in the third quarter, though funds and institutions still hold the bulk of the freely traded shares.

Where the Street Stands

Despite the doubts voiced in some corners, optimism prevails on Wall Street. The analyst consensus rates the stock a Moderate Buy with an average price target of USD 502.53. The clear majority of experts still recommend buying, with only a minority counseling patience or an exit. Barclays’ fresh Overweight call fits that pattern — a bet that the procedure engine keeps humming and that the sector’s fundamentals, and its investors’ mood, improve from 2027.
